I Lost My Childs Shot Record Please Help!!!!!?

I lost my child's shot record, I put it in the same place I always put it and it wasn't there and I have been looking everywhere for it and cannot find it. How can I get another shot record, I have all of his shots written down in his baby book so it won't be hard to fill it back out.
